A packet with relevant information will be sent to you. Please return the applicable forms so they can be incorporated in your records. Your initial appointment will consist of an evaluation and gathering appropriate information for an accurate diagnosis. This information will be studied so possible treatment options can be determined. In the event of an emergency or urgent care, treatment may be performed the same day as the evaluation. However, a complex medical history or complex treatment plan will require time to develop appropriate treatment options. Often, additional appointments may be necessary to gather pertinent, diagnostic information. A consultation appointment will be made to discuss different options of treatment after a thorough diagnosis has been made.

IMPORTANT: A parent or guardian must accompany all patients under 18 at the consultation visit.

Please alert the office if you have a medical condition that may be of concern prior to dental treatment (i.e. diabetes, high blood pressure, artificial heart valves and joints, rheumatic fever, etc.) or if you are on any medication (i.e. heart medications, aspirin, anticoagulant therapy, etc.).

X-Rays

If your dentist or physician has taken x-rays, you may request that they forward them to our office. If there is not enough time, please pick them up and bring them to our office. If additional films are necessary, x-rays will be taken at our office.
